I own and operate a graphic design studio and have gotten to a point where everything is dead-on from input to final output.
First there are a few things you need to except.
There are many factors such as what color space you are working in, your proofing device as well as your final output device.
1) Color is perceptual so even an older or less expensive monitor is capable of rendering color acurately (perceptualy acurate that is).
2) color output devices are simalr CRT monitors are not. AKA you can usualy use a "canned" printer profile with reasonably accuracy. but "canned monitor are total crap with out exception.
3) Beware there are a lot of so-called experts out there and most are clueless and will lead you down some dangerous roads.
That said a good source for this imformation is creativepro.com search for color management or ICC profiles. David Blattner, Bruce Frazier and Andrew Rooney are the top guys in the field.
