Nick,
As you said, There are a lot of variables. I would start be reinstalling Distiller. Also check Adobe's web site, there have been a couple of updates posted there. Also, I would write the pdf using the program in which the file was created. You have that program open at that time so why take more time and use more resources by opening another program to write the same type of file.
